Stroke Scale Type and Stroke Scale Score Requirement

At least one Stroke Scale Type and one Stroke Scale Score must be documented on Stroke/CVA related patients using the Stroke Scales or Stroke Scale Power Tool.   This does not change any requirements outlined in each GMR company specific policies, protocols, or related to proper documentation of the care rendered.

Validation Rules for Stroke Scale Score and Stroke Scale Type  

This requirement is satisfied by documenting the Stroke Score Type and Stroke Score in one of the following Power Tools:            

Stroke Scales (Multiple)

Stroke Scales (Cincinnati)

NOTE:  Worksheets do not satisfy this requirement.
