Patient Address info is required and I don't know the information. How do I satisfy the requirement?

If you do not know any information about the patient address you can satisfy the requirement by selecting the city where you made contact with the patient.    

If you know the Postal Code then enter it and click Set From Postal Code.   This will populate the required data.

If you do not know the Post Code then select Location Lookup and search for the City.

Left click to select the correct city and click OK.   The data will then populate to satisfy the requirements:
