How to Complete a Respiratory Medical Questionnaire

New hires will be required to complete a Respiratory Medical Questionnaire.

1. Navigate to the Forms section. There are multiple ways to get to the forms section. On the left side panel, click on Submit a Form. You can also click on the red “Forms” button.

 2.  Navigate to “Respiratory Medical Questionnaire” and click “Start Form”.

 3. Complete the form and click “Submit Form”.
